Residental Application Agreement
In consideration of Taylor West Weber Water Improvement District (“TWWW”) furnishing culinary (drinking) water to the service address listed above, the Owner agrees as follows:
· I/we hereby apply for culinary (drinking) water service from Taylor West Weber Water, (TWWW) to the service address above.
· I/we agree to pay all bills and all other applicable charges and assessments of TWWW. Any amount which is not paid when due, as specified on the invoice or other statement, shall receive a monthly penalty.
· In the event of failure to pay for services at the above-mentioned address, I/we hereby authorize TWWW to shut off the water to this property at its election and without further notice. TWWW may record a lien on the service address property for past due fees.
· I/we further agree to be bound by all the rules and regulations of TWWW for the control of its water system.
· I/we specifically agree to pay all reasonable attorney fees and court costs in the event legal action is taken to collect on the account. I/we further agree to pay an additional amount of 40% of the balance if the account is referred to a collection agency or attorney for collection.
· Written shutoff and other notices may be delivered solely to the service address and TWWW has no obligation to ensure that the Owner receives actual notice of an intention to disconnect service.
It is the Owner’s responsibility to ensure TWWW has the Owner’s current mailing address and other contact information.
· If legal title to the service address is transferred to a new owner, Owner agrees to give notice of such transfer to TWWW. Until such notice is given, Owner’s liability hereunder shall continue in full force and effect.
· In response to State and Federal regulations, TWWW has a Back-flow Prevention Program.
(a) All new connections will have a back-flow (double check) valve installed in the meter box.
(b) Back-flow devices create a closed system which may create thermal expansion. Thermal expansion means that as water is heated or cooled, it will expand and contract. This is usually compensated for with a pop-off valve on the hot water tank or installation of an expansion tank. Any existing connections that do not have a back-flow device installed, TWWW can at any time install the back-flow device and notify Owner. Owner agrees to install what is necessary to make sure there is not a problem with thermal expansion.
(c) This paragraph (4) is to inform you of the potential problem that may be created with a closed system and by signing below, Owner acknowledges reading this and understanding the potential problem and agrees that TWWW will be held harmless of any problems arising from the installation of the back-flow device.
· Meter boxes must always remain accessible to TWWW employees. Anything laid down other than grass around the meter box will require the ring and lid to be changed to an 18” drive-over lid and ring at the Owner’s expense. TWWW can supply grade rings to raise the ring and lid if needed after landscaping, also at the Owner’s expense. Should the meter box need to be dug up for any reason, it is the Owner’s responsibility to replace any concrete, bricks, etc., that might have been placed around it.
· The Owner is responsible for all damage occurring at the meter box including but not limited to, the ring and lid, radio, antenna and register. TWWW will replace the damaged equipment and bill the Owner.
· TWWW provides culinary (drinking) water. If a box is marked below for one of the pressurized secondary water providers, Owner agrees to only use them for their sprinkling system and/or outdoor irrigation. Cross-connections are illegal, and swing connections are not allowed. TWWW will impose a $1,000 fine and disconnect culinary water service if found to be in violation.
· If a box is not marked, TWWW has no obligation to provide water for outdoor use and may, at any given time, restrict or deem necessary not to allow any outdoor use for any purpose
